Certificates of Insurance (COIs)


TYPES OF CERTIFICATES OF INSURANCE

  • NAMED INSURED / SIMPLE PROOF OF INSURANCE. This type of certificate is used for a simple, standard proof of insurance for a US Club Soccer Organization Member (ex: a club) with no Additional Insured status provided. The requesting entity is listed as the Certificate Holder and the US Club Soccer Organization Member is listed in the description of operations.

  • ADDITIONAL INSURED – FIELD/FACILITY OWNER. This type of certificate is issued to a field/facility owner used by a US Club Soccer Organization Member. The field/facility owner is listed as the Certificate Holder and as an Additional Insured, and the US Club Soccer Organization Member is included in the description of operations.

  • ADDITIONAL INSURED – NON-FIELD/FACILITY OWNER. This type of certificate is issued to an Additional Insured who is not a field or facility owner. The requesting entity is listed as the Certificate Holder and as an Additional Insured, and the US Club Soccer Organization Member is included in the description of operations. This is not common, and additional review and approval is necessary.

REQUEST A CERTIFICATE OF INSURANCE (COI)

This form is for US Club Soccer Organization Members to use to request a certificate of insurance (COI).

NOTES:

  • Submit a separate form for each COI needed.

  • Turnaround time is usually within three business days, but endorsement or other requests may require the involvement of our insurance broker or carrier and generally take longer to issue.

  • If approved, the COI will be issued to the US Club Soccer Organization Member regardless of who is listed as the Certificate Holder.

  • This certificate and the corresponding insurance coverage is valid only for US Club Soccer programming. Coverage does not extend to non-US Club Soccer programming.

  • This form must be submitted by an individual who has administrative rights within the applicable US Club Soccer Organization Member’s GotSport account.

  • For Youth Programming (U-20 and below) | Aug 1, 2024 – Jul 31, 2025
  • For Adult Programming (U-21 and over) | Jan 1 – Dec 31, 2024
