The parent/guardian (in the case of a minor player) or adult player is to complete this form as part of the US Club Soccer player registration process.
For the 2023-24 and 2024-25 registration years, the previous version of this form will continue to be accepted if it was previously completed (and still valid) or already in use with a member org’s internal registration process. In all other situations, this new version should be used. In the future, it is anticipated that completion of Form #R002 will be required on an annual basis.
This form is to be retained by the US Club Soccer member organization for at least five (5) years or until the player’s 18th birthday, whichever occurs last, and be provided to US Club Soccer upon request.

The Submit a Safeguarding & Compliance Report web page includes information regarding the requirement reporting suspected child abuse to law enforcement pursuant to the mandatory reporting provisions of federal law/the SafeSport Act, as well as information and a form to report another concern, referee assault or abuse, or other incident to US Club Soccer.

US Club Soccer requires submission of a list of attending teams and contact information to tournament@usclubsoccer.org within 10 days of the tournament’s conclusion. Minimally, this shall include coach and manager names, e-mail addresses and states of all attending teams. This template is provided as a courtesy.
Supplemental tournament insurance is offered to hosts of US Club Soccer-sanctioned tournaments in order to obtain liability and accident insurance coverage for all non-US Club Soccer participating teams. (Teams using US Club Soccer passcards already have insurance coverage.) This simplifies attendance for teams, especially if a number of non-US Club Soccer teams are not able to obtain insurance from their state association or other U.S. Soccer member organization.
